With effect from 1st December 2014 the NHS friends and family test will be introduced in all GP Surgeries.

Please click here to download the form.  Once you have completed, please email to whccg.StAndrewsSurgery@nhs.net

We also have a Friends & Family box at reception where you can fill in a form and post it in the box.

Using Patient Access at St Andrew's Surgery, you can now book your appointments online, 24 hours a day, 365 days a year, all from the comfort of your own home.

If you are a patient at St Andrew's Surgery and you wish to start using this service, please visit the surgery and bring in proof of identification, i.e. passport, drivers license or recent utility bill.  We will then set you up on the system and print off a Patient Access Registration Form which you need in order to finalise setting up your account.
